Missouri State Immunization Requirements

All students must present documentation of month, day, and year of each immunization.

Students must present immunization record to school, and all immunizations must be up-to-date before being permitted to attend classes. To remain in school, students "in progress" must receive immunizations as soon as they become due. Religious and Medical exemptions will be allowed with proper documentation. Please see your school nurse or health department for the appropriate forms.

K	4 - 5 Doses Last dose on or after 4^th birthday and last dose of pertussis before 7^th birthday.	3 Doses Last dose on or after 4^th birthday. If a combination on IPV/OPV is received, 4 doses are required	2 Doses On or after 1^st birthday, 28 days between the two doses.	3 Doses
1 - 4	4 Doses Last dose on or after 4^th birthday and last dose of pertussis before 7^th birthday. Pertussis is required for all students 6 years of age and younger	3 Doses Last dose on or after 4^th birthday. 5 - 11	3 Doses Td booster required 10 years after last dose of DTaP, DTP, DT, or Td.	3 Doses Last dose on or after 4^th birthday. If a combination on IPV/OPV is received, 4 doses are required	2 Doses On or after 1^st birthday, 28 days between the two doses.	3 Doses
12	3 Doses Td booster required 10 years after last dose of DTaP, DTP, DT, or Td.	3 Doses Last dose on or after 4^th birthday. If a combination on IPV/OPV is received, 4 doses are required	2 Doses On or after 1^st birthday, 28 days between the two doses.	N/A
